Patient Registration
Welcome to Dy Dermatology Center. When you call our office and schedule your first visit, our coordinator will ask you for your demographic information including email address and insurance information. We will then email you a “link” to our patient portal. Please create your custom log in within 48 hours of receiving the link, or the link will expire. After you have established your login, you can go into the patient portal up to 48 hours prior to your appointment to complete your health history. Completing your health history in advance of your visit greatly improves efficiency, accuracy, and thoroughness at the first visit. If you are unable to log into our patient portal, you may download our patient registration form, complete, and bring to your visit. Please click here.
On the day of your appointment, please arrive 10 minutes prior to your scheduled appointment time and present the following information to our coordinator:
Financing Options
Financing With CareCredit
CareCredit is the nation’s leading healthcare financing program. For nearly 30 years, CareCredit has been providing patients with finance options for procedures that are typically not covered by traditional health insurance. Applying for financing through CareCredit is easy – you can apply online and credit decisions are determined immediately so there is no waiting for an approval letter!

Patient Hair Form
If you have had bloodwork (including thyroid and ferritin) performed within the past six months, please bring in the results for Dr. Dy to review. Also, we do have an additional form for you to complete and bring to your visit. Please click here.
Appointment Policy
We do ask all patients to arrive at least 10 minutes prior to their scheduled appointment to complete the check-in process. Patients that arrive 11 minutes or more after their scheduled appointment will be asked to reschedule.
In order to maintain the highest standards of care, we require 48 business hours-notice of cancellation of your appointment. Less than 48 business hours does result in a cancellation fee of $50. Cancellation of a procedure with less than 48 business hours-notice incurs a $150 fee.
Cancellation of a cosmetic procedure with less than 48 business hours-notice, incurs a $150 fee.
